The MyMoffitt Patient Portal is a free, secure gateway into your personal health information at Moffitt Cancer Center. Click here to log into your account.
With this tool, you can:
1. View upcoming appointments and appointment instructions.
2. Request appointment changes or cancellations.
3. View/edit your personal information.
4. Take the Patient Medical Questionnaire online, prior to your appointment.
5. Pay your Moffitt bill online.
6. Find helpful information on cancer survivorship and other patient-related topics.
Register for this free Moffitt service today!
Before you start, please make sure:
- You are a Moffitt Cancer Center patient and have been given a medical record number (MRN).
- You have an email address. You can get one free at these web sites - Gmail.com, Hotmail.com, AOL.com or Yahoo.com.
- You have a high-speed connection to the Internet. If you do not have Internet access at home, most public libraries offer this service to the public.
- If you do NOT have a patient portal account, click here for instructions on how to create one.
Once you have logged into the Patient Portal, you will be able to take the Moffitt Patient Questionnaire.
Should you have any questions or difficulties logging into the Patient Portal, call Cancer Answers at 1-888-MOFFITT (1-888-663-3488) or complete a portal help request form. Form requests will be responded to within one hour during business hours (Monday through Friday, 8 a.m. to 5 p.m.).
